Patrons request.

You have many options for accepting patron requests:

Paper form (link to an example)
Online form that delivers information to an e mail account or IMS
From ILL forms embedded in research databases (Ovid, Ebsco, CSA, etc.)
From OCLC FirstSearch databases to review or produced through OCLC DirectRequest

Make sure the form you use includes all the information you require to place the request.

Books and other returnables: title, author, imprint
Articles: article title and author, journal title, volume, number, date and pages
Microfilm reels: reel numbers or dates

Other considerations:

What information do you need to authenticate your patrons? (name, address, e mail, affiliation, ID number) (ILLiad and ClioAdvanced require a patron log in order to place requests)
What information will be required for statistics (patron status, department)?
What information do you need in order to communicate with your patrons? (e mail, phone, mail?)
